Metal Nametags

If you are reordering nametags, you do not need to use this form,

simply e-mail orders@nametagsource.com and include your name or order number,

list of names to be made, backing types and shipping/billing information.

Step 3:

Tell us what your first tag will look like.  This nametag will be the template for the rest of the nametags. For example, If you would like a 1 line nametag, you need only fill in line 1.  If you would like a three line nametag, you will need to fill in all three lines.  You may also choose different fonts for each line.  For example, you may want the company name in a different font than the name.

Including the nametag that you designed above, please enter how many total nametags you are ordering.

Enter your list of names.  If you are only ordering one nametag,

Please list any special instruction such as "Line 1 should be slightly bigger than line 2"
